📋 Description Make home visits to clients in designated geographic territories. Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients Develop a treatment plan based on evaluation, physician’s orders, and goals. Continually assess and revise the care plan; participate in multidisciplinary team planning. Educate patients and families on rehabilitative care and activities to promote health and Document observations and interventions using a state-of-the-art touch pad tablet. 🎯 Requirements Current license as a Speech Language Pathologist in North Carolina.
