Medical Social Worker, Hospice

📋 Description Make home visits to clients in designated geographic territories in Salisbury, NC area. Assist the care team in understanding social and emotional factors related to client health Provide clinical social evaluations and plan interventions based on evaluation findings. Identify community resource referrals for client and family needs. Advocate for client and family to navigate the community system. Participate in the development of the total plan of care with BAYADA clinicians. 🎯 Requirements Masters in Social Work with at least 2 years of community-based experience. Accreditation by the Council on Social Work Education (CSWE). Home care experience a plus. Strong English communication; ability to travel to cases as assigned.
