📋 Description Lead efforts to improve HEDIS medication adherence and related quality measures. Analyze adherence data; identify gaps; implement targeted interventions. Collaborate with providers, care managers, and stakeholders to promote adherence. Conduct MTM sessions; educate patients and address barriers to adherence. Work with patients to resolve issues like cost, side effects, and complex dosing. Use motivational interviewing to encourage adherence to regimens. 🎯 Requirements PharmD degree from an accredited school. Active pharmacist license in TX, PA, AL, or TN. 2+ years in HEDIS-related adherence, quality improvement, or population health. Strong knowledge of HEDIS measures related to adherence. Proficiency with EHR systems and data analysis tools. Excellent communication, problem-solving, and organizational skills.
