📋 Description Care coordination for MSHO/MSC+ members in the community and assisted living. Work from home with 50-70% travel in the St. Paul, MN area. Coordinate face-to-face visits and annual assessments (PCA where needed). Develop individualized care plans and connect members with resources. Support transitions of care and collaborate to meet quality measures.
