📋 Description Conduct site visits, vitals, assessments, med counseling, document progress Ensure quality, individualized health care management for optimal health Ensure compliant medical documentation per licensing and policy Coordinate with direct support staff to support health care needs Attend support plan meetings to advise on patient monitoring Train and delegate nursing tasks (insulin, G-tube, catheter, wound care) with supervision 🎯 Requirements 2+ years RN experience Active Oregon RN license Valid driver's license and insured vehicle Experience in long-term care or IDD preferred Strong organizational skills and scheduling Solid computer and documentation skills 🎁 Benefits Hourly rate starting at $45/hour (tenure increases) Part-time 30 hours/week; no weekend shifts Medical, Vision and Dental insurance eligibility for 30+ hrs 401(k) plan with up to 3% employer match Paid Time Off (PTO) for eligible employees Growth and Development Opportunities
