📋 Description Make home visits to clients in designated territories. Evaluate, diagnose and provide speech therapy for communication or swallowing issues. Develop a treatment plan based on evaluation, orders and goals. Continually assess and revise the speech therapy care plan with the multidisciplinary team. Educate patients and families in rehabilitative care for independence.
