Speech Language Pathologist, Home Health

📋 Description Make home visits to clients in designated geographic territories. Perform evaluations to assess and treat speech, language, or swallowing difficulties. Develop treatment plans based on evaluations, orders, and goals. Continuously assess and revise care plans with the multidisciplinary team. Educate patients and families on rehabilitative care for safety and independence. Document care observations and interventions on a touch pad tablet. 🎯 Requirements Current VA license as Speech Language Pathologist. CCC-SLP from ASHA, or equivalent. Education and work exp toward ASHA certification, including awaiting exam. In process of completing requirements for certification.
