Hospice RN Case Manager

📋 Description Complete comprehensive nursing assessments evaluating physical, emotional, and psychosocial needs. Develop and manage individualized care plans with patients, families, physicians, and the team. Monitor patient progress and adjust care plans to maximize comfort and quality of life. Serve as the primary clinical resource for patients and families throughout their hospice journey. Assess and manage pain and end-of-life symptoms; educate on medications and progression. Coordinate care with nursing, social work, chaplaincy, aides, and physicians; on-call. 🎯 Requirements Current, unrestricted RN license in the state of practice. Minimum one year of nursing experience. Valid driver's license and ability to travel throughout service area. Hospice, palliative care, home health, or case management experience preferred. Strong clinical assessment and care planning skills.
