Outreach Specialist / Care Coordinator - Hampton Roads, Virginia

📋 Description Initiate outreach to new patients and connect to resources. Foster trust and improve patient engagement through empathetic communication. Review patient data from EHRs to optimize care and prevention. Communicate with primary care providers via multiple channels (EHR, in-person). Be the primary day-to-day contact and build collaborative provider relationships. This is a hybrid role requiring some on-site work in facilities. 🎯 Requirements Excellent verbal and written communication skills. Empathetic, credible communication with physicians and patients. Highly organized, self-motivated; customer service or sales experience. Experience with Electronic Health Records (EHRs). Comfort with Google Workspace and other tech platforms.
