📋 Description See a client from hospital discharge to a safe, comfortable home life. Perform diagnostic tests and measurements (mobility, strength, balance, safety). Develop and implement individualized care plans, including manual therapy and gait training. Continually assess and revise the PT care plan with nursing in a multidisciplinary team. Implement wound care with evidence-based dressings and topical agents. 🎯 Requirements Active and current license to practice as a Physical Therapist (PA). Home health experience welcome; training provided. Ability to travel to cases as assigned. Calm, reassuring presence with strong listening and communication.
