Speech Language Pathologist - Senior Living Visits

📋 Description Make home visits to clients in designated territories. Perform evaluations to diagnose and treat speech, language, or swallowing issues. Develop a treatment plan based on evaluation, orders, and goals. Continually assess and revise the care plan; join the multidisciplinary team. Educate patients and families in rehab activities for health and independence. Document observations and interventions using EMR on a tablet. 🎯 Requirements Current PA licensure as Speech Language Pathologist. ASHA CCC-SLP or equivalent for exam eligibility. Eligible for ASHA exam with required education/experience. Education/experience to qualify for certification or in progress. Solid computer skills; EMR experience preferred.
