📋 Description Support care delivery within the Evergreen Aligned (EGA) model with APPs. Ensure effective execution of individualized CKD/HTN/diabetes/CHF care plans. Provide education, remote monitoring, and medication management. Coordinate care through labs, referrals, and follow-up communications. Conduct pre-visit planning and reinforce APP-directed plans. Promote patient engagement and outcome improvement in a virtual setting. 🎯 Requirements Active, unrestricted RN license. 2+ years of clinical experience in CKD, diabetes, or hypertension. Experience with EHR (electronic health records). Excellent communication; bilingual as applicable. Ability to work remotely with minimal supervision.
