📋 Description Visit clients in their home per care plan Case management, care plans, assessments, and wound care Progress clients from discharge to achieving home goals Educate patients and families on care Weekend per diem schedule 🎯 Requirements Active RN license One year of clinical experience preferred Home health experience welcomed, but not required. We will train you!
