Job Overview
The Health Coach provides individualized health coaching to RHM and RHI patients while supporting patient engagement, communication, scheduling, and care coordination. This role manages a personal appointment calendar and serves as a responsive point of contact for patients participating in health coaching services.
Responsibilities
Health Coaching and Patient Engagement
- Provide health coaching appointments to RHM patients.
- Support patients in developing and working toward individualized health and wellness goals within the scope of the Health Coach role.
- Develop strategies to engage and encourage appropriate patients to participate in health coaching services.
- Follow up with patients regarding questions, needs, and next steps related to health coaching.
- Maintain timely, professional, and compassionate communication with patients.
Patient Coordination and Administrative Support
- Coordinate patient scheduling, inquiries, and communication related to health coaching services.
- Manage a personal appointment calendar and maintain accurate scheduling information.
- Respond to and coordinate incoming and outgoing patient inquiries and communications within 24 hours.
- Collect, organize, and appropriately file patient forms and documentation.
- Manage incoming and outgoing faxes and emails related to assigned responsibilities.
- Maintain accurate and organized patient information in accordance with RHM policies and applicable privacy requirements.
- Collaborate with RHM team members to support continuity of patient care and an efficient patient experience.
- Perform other related duties as assigned.
Qualifications
Required
- Completion of a health coaching, nutrition, or related certification program.
- Alignment with the mission of RHM, including recognition of the dignity of every human person from conception through natural death.
- Excellent communication and interpersonal skills.
- Exceptional attention to detail.
- Strong organizational and time-management skills.
- Ability to prioritize and efficiently manage multiple responsibilities in a fast-paced environment.
- Strong problem-solving and analytical skills.
- Working knowledge of general administrative and clerical procedures.
- Intermediate computer and software proficiency.
- Ability and willingness to learn new technology and software platforms, including Microsoft Teams, athenahealth, and RingCentral.
Preferred
- Experience in a healthcare, medical office, health coaching, nutrition, or patient-facing environment.
- Working knowledge of healthcare insurance processes and medical terminology.
Benefits
This is a part time position so is not eligible for full a full benefit package. The following is a list of our part time staff benefits:
- 24hrs of PTO accrued every 500 hours worked
- Some part time staff are eligible to participate in our 401K program after one year of employment.
