Physical Therapist, Per Diem Weekends, Home Health

📋 Description

• Visit clients in their homes in designated geographic areas
• Perform diagnostic tests and measures (mobility, ROM, transfer, balance, gait)
• Implement wound care and protective interventions with evidence-based dressings
• Develop and revise individualized PT care plans, including manual therapy, gait training, balance
• Document observations and interventions using a state-of-the-art touch pad tablet
• Collaborate with nursing and the interdisciplinary team for comprehensive care

🎯 Requirements

• Active and current PT license
• One year of clinical experience preferred
• Home health care experience welcomed, but not required (training provided)
• Ability to travel locally to cases as assigned
