Advanced Care Provider- Virtual Care - Seasonal Part Time

📋 Description

• Triage incoming patients needs, assess their concerns and determine the best course of action
• Review medical inquiries from Sollis patients that arrive via email, text or phone, monitor
• Follow up with patients after they have been seen by a Sollis Health provider to assess their
• Follow up on send-out labs, imaging reports, monitor portals and navigator tasks.
• Document clinical evaluation and treatment plans in Sollis’ EHR
• Assist patients with hospital admissions and transfers, as needed.

🎯 Requirements

• Bachelor's Degree, required. Graduate of an accredited PA or NP program, required. If NP, RN degree
• 3+ years of Physician Assistant or Nurse Practitioner experience in emergency medicine, urgent
• Must have active license(s) in one or more of the following states: NY, CA and FL. Multiple state
• DEA license required
• NCCPA or Family Nurse Practitioner certification required
• ACLS and PALS certification required
