Licensed Practical Nurse (LPN) – Home Health Visits

📋 Description

• Provide skilled nursing visits to clients in Hudson County, NJ.
• Administer medications, wound care, treatments, and physician-ordered nursing interventions.
• Monitor client conditions and report changes to the Clinical Manager.
• Complete comprehensive nursing assessments as needed.
• Document physician orders accurately and timely.
• Coordinate care with physicians and the interdisciplinary team.

🎯 Requirements

• Current New Jersey LPN license.
• Minimum one year of recent, verifiable clinical LPN experience.
• Graduate of an accredited nursing program.
• Home health experience preferred but not required.
• Ability to work independently and manage time effectively.
• Strong communication, organizational, and interpersonal skills.
