📋 Description

• Per diem Medical Social Worker performing home health visits in Marion County.
• Join a multidisciplinary team delivering skilled nursing and rehabilitative care at patients' homes.

🎯 Requirements

• Master of Social Work (MSW) required.
• Licensed LMSW or LISW by Indiana Board of Social Work Examiners.
• 2+ years of community-based experience preferred; home care a plus.
• Strong English communication and independent work skills.
• Proficient computer skills; EMR experience preferred.
