Occupational Therapist (OT), Per Diem, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate patients’ functional status and ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects to plan care.
• Assess home hazards and adapt environments for independence.
• Identify equipment needs and environmental adaptations.
• Revise the OT care plan with the care team; educate patients and families.

🎯 Requirements

• Missouri Occupational Therapist license.
• At least one year of OT experience.
• Graduate from an accredited OT program.
• Ability to work independently and manage time; strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• We will train you; home health experience welcome but not required.
