Physical Therapist (PT), Full Time, Home Health

📋 Description

• Visit clients in their homes within designated territories
• Perform diagnostic tests and measurements (mobility, gait, balance, strength)
• Implement wound care and protective interventions as indicated
• Develop and revise individualized PT care plans; include manual therapy and gait/balance training
• Educate clients and families to promote health, safety, and independence
• Accurately document observations and interventions using a tablet

🎯 Requirements

• Active and current PT license
• One year of clinical experience preferred
• Home health care experience welcomed, but not required; training provided
• Ability to travel locally to cases
