Registered Nurse - Home Health (Full Time)

📋 Description

• Visit clients in their homes for RN care.
• Case manage and develop care plans.
• Deliver skilled care: assessments, med mgmt, wound care.
• Progress clients from discharge to goals at home.
• Provide education to patients and families.
• Join a larger interdisciplinary care team with support.

🎯 Requirements

• Active and current RN license.
• One year clinical experience preferred.
• Home health experience welcomed but not required; training provided.
• Ability to travel locally to cases as assigned.
