Registered Nurse, RN - Home Health

📋 Description

• Provide skilled nursing and rehabilitative care in patients' homes
• Integral member of a multi-disciplinary team to support home health goals
• Perform head-to-toe assessments and identify changes in patient condition
• Triage and manage urgent clinical situations independently in the home
• Medication reconciliation, patient teaching, and safety monitoring
• Wound assessment, care, and documentation; travel within the coverage area

🎯 Requirements

• Current nursing license in good standing in Colorado
• Graduation from an accredited nursing program
• At least one year of adult/geriatric clinical experience in acute care, home health, ER, ICU
• Medicare Certified Home Health and OASIS documentation experience strongly preferred
• Ability to work independently and make real-time clinical decisions
• Flexible and able to travel throughout the office coverage area
