Care Coordinator, RN (MSHO)

📋 Description

• Hybrid role; work from home with local visits
• Travel to patients in their homes and facilities
• Manage DHS and CMS requirements for MSHO/MSC+ members
• Coordination across care teams to meet quality measures

🎯 Requirements

• Minnesota Registered Nurse license
• One+ years in geriatric/certified disabled case management or related settings
• Driver's license required
• Ability to work independently and in diverse environments
• Excellent communication and documentation skills
• New grads encouraged to apply (LSW/LMSW preferred for MN social work)
