• Manage an assigned patient’s caseload
• Adhere to a revolving monthly patient’s service and compliance standard
• Direct point-of-contact for enrolled member questions, inquiries or concerns
• Experience with data input, case review and building rapport with patients and caregivers
• Adept at multitasking, excellent communication and organization
• Collaborate with Care Coordinators in scheduling home visits for efficiency

🎯 Requirements

• 1+ years of experience in field-based roles
• Capable of assessing patient health risks and identifying hazards in the home
• Motivation to meet case management targets
• Excellent written and verbal communication skills
• Excellent time management, critical thinking, active listening; multitasking
• Own reliable transportation and ability to travel daily
