Outpatient Physical Therapist

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility/ROM, transfer status, gait, strength
• Develop and implement individualized care plans with manual therapy, gait training, balance, and
• Continually assess and revise the PT care plan; collaborate with nursing in multidisciplinary plans.
• Educate clients and family members in rehabilitative care to promote independence.
• Accurately document observations, interventions, and evaluations using EMR tablets.

🎯 Requirements

• Current VA Physical Therapist - PT license.
• Graduation from an APTA-accredited program or equivalent.
• Ability to work independently and manage time; strong interpersonal and organizational skills.
• Solid computer skills; prior EMR experience preferred.
