Care Coordinator, LSW (MSHO)

📋 Description

• Hybrid role with home visits and local travel in Minnesota
• Manage DHS and CMS requirements for MSHO/MSC+ members
• Provide care coordination to help members access medical care, waiver services and community
• Collaborate with medical teams to meet quality measures and value-based goals
• Support transitions of care and safe discharges with follow-up planning
• Work primarily from Brooklyn Center, MN with remote work 30-50%

🎯 Requirements

• Current MN Licensed Social Worker (LSW, LMSW, LISW, or LICSW) – new grads encouraged to apply
• One+ year experience with geriatric or certified disabled populations in case management/care
• Valid driver’s license
• Ability to work independently; strong communication and customer service skills
• English language proficiency
• Knowledge of Elderly Waiver and community resources
