Care Manager - 1099 Contract $50 per visit (Indianapolis, IN / Field-based)

📋 Description

• Orient new patients and assess home care needs
• Train and supervise caregivers to ensure quality care
• Coordinate care with patients, families, and staff
• Travel within Indiana to support patients and perform visits
• Complete orientations and initial assessments during onboarding
• Engage in interdisciplinary meetings to improve case outcomes

🎯 Requirements

• 2+ years of experience with frail or elderly disabled adults and caregivers
• Hands-on experience in home care
• Knowledge of ADL/IADL assessments; valid driver’s license; insured vehicle
• Excellent verbal/written communication and organizational skills
• Proficient in Microsoft Office; Salesforce experience a plus
• Ability to travel statewide in Indiana and work collaboratively
