📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, transfer, stability, ambulation
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy, gait training, balance, and
• Continually assess and revise care plans; coordinate with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care to promote health and independence.

🎯 Requirements

• Active VA Physical Therapist license.
• Graduation from an APTA-approved program or equivalent (per transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
