Medical Social Worker, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Assist our team in understanding social and emotional factors related to clients’ health problems.
• Provide clinical social evaluations and plan interventions based on findings.
• Identify community resource referrals for patient and family needs.
• Advocate for clients and families to navigate the community system.
• Participate in developing the total plan of care with the multidisciplinary team.

🎯 Requirements

• Must be licensed by the VA Board of Social Work Examiners at LMSW or LISW level.
• Master of Social Work with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education (CSWE).
• Home care experience is a plus.
• Strong English communication; able to work independently and manage time.
• Strong interpersonal skills; solid computer skills; EMR experience preferred.
