📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, transfer status, balance, strength
• Develop and implement individualized care plans (manual therapy, gait training, balance, other
• Continuously assess and revise PT care plan; collaborate with nursing in multidisciplinary care
• Educate clients and family on rehabilitative care to promote health and independent living.
• Accurately document observations and interventions using a touch pad tablet.

🎯 Requirements

• Current VA Physical Therapist - PT license.
• Graduate from APTA-approved program or CAAHEA-equivalent (as indicated by transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
