Physical Therapist- Home Health

📋 Description

• Provide home health visits in designated Warrenton area territories
• Assess patient mobility, gait, balance, strength, and endurance
• Develop and implement individualized care plans and interventions
• Educate clients and families on rehabilitative care for independent living
• Document observations and interventions using EMR
• Collaborate with nursing and multidisciplinary team

🎯 Requirements

• Active VA Physical Therapist license
• Minimum of 1 year PT experience
• Ability to work independently; strong organization and problem solving
• Good interpersonal skills; comfortable with EMR systems
• Strong manual therapy, gait, balance, and safety assessment skills
• Graduation from an APTA/CAFHEA accredited program
