📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, transfer status, strength, balance
• Implement wound care interventions and promote wound repair with evidence-based dressings.
• Develop and implement individualized care plans (manual therapy, gait training, balance, other
• Assess and revise PT care plans; coordinate with nursing in multidisciplinary plan.
• Educate clients and families to promote health, safety, and independent living.

🎯 Requirements

• Active VA Physical Therapist license.
• Minimum 1 year PT work experience.
• Graduate from an APTA-approved program (or CMAA/AMA equivalent).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
