📋 Description
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence
- Partner with physicians, specialists, and interdisciplinary teams to order individualized care
- Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate.
- Support quality-improvement and REMS initiatives.
- Provide recommendations to operations by supporting teammates as needed with complex prior
🎯 Requirements
- Hold a Doctor of Pharmacy (PharmD)
- Must be licensed pharmacist in Washington
- PGY2 Oncology Residency or PGY1 Ambulatory Care with 3+ years oncology experience or 3 years
- Strong communication, organization, and tech skills
- Empathy, integrity, and a team-first mindset
