📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements: mobility, transfer status, ambulation, strength
• Implement wound prevention and repair with evidence-based dressings.
• Develop and implement individualized care plans: manual therapy, gait training, balance, other
• Assess and revise care plans; collaborate with nursing on the care plan.
• Educate clients and families on rehabilitative care and safety.

🎯 Requirements

• Current VA Physical Therapist license.
• Graduate from APTA-approved program; transcript/diploma.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
