Job Description
📋 Description
- Deliver personalized nursing care to PACE participants.
- Assess physical, psychosocial, and behavioral domains.
- Coordinate care with the interdisciplinary team (IDT).
- Educate participants, caregivers, and team members on care plans.
- Manage end-to-end care coordination and transitions of care.
🎯 Requirements
- Unencumbered California RN license required.
- 2-4 years clinical experience with medically complex/older adults.
- Graduate or BSN preferred; accred nursing education.
- Wound care, IV therapy, phlebotomy, chronic disease mgmt experience.
- CPR/BLS current or ability to obtain within 30 days.
- Excellent communication with participants, families, and team.
🎁 Benefits
- Base salary $121,000 - $141,000 per year (est.).
- 5% annual bonus target; additional incentive pay possible.
