📋 Description

• Make home visits to clients in designated geographic territories.
• Assess mobility, range of motion, transfer status, gait, strength, endurance, balance, and safety.
• Provide wound care and integumentary interventions with evidence-based dressings and topical agents.
• Develop and implement individualized care plans including manual therapy, gait training, balance

🎯 Requirements

• Current VA Physical Therapist license.
• Graduation from an APTA/CAAHEA-accredited program (transcripts/diploma acceptable).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
