Care Manager

📋 Description

• Field-based care management in GA (Bibb/Lamar area)
• Perform patient orientations and assessments
• Train caregivers for quality home care
• Coordinate with families and stakeholders
• Travel within Georgia for assigned cases
• Support patient independence at home

🎯 Requirements

• 2+ years in frail/elderly caregiver support a plus
• Strong communication and organizational skills
• Excellent problem-solving and critical thinking
• Hands-on home care experience preferred
• License as required for travel in GA
• Salesforce experience a plus; MS Office skills required
