• Provide comprehensive medication management including reconciliation and dose optimization.
• Collaborate with physicians and teams to order customized care plans under a CPA.

• Support QA, REMS initiatives; improve workflows and educate patients.

🎯 Requirements

• Doctor of Pharmacy (PharmD).
• Licensed pharmacist in Washington.
• PGY2 Ambulatory Care Residency OR PGY1 plus ambulatory CPA OR 3+ years ambulatory practice.
• Strong communication, organization, tech skills.
• Empathy, integrity, team-first mindset.
