Role Overview: Nurse Practitioner, Neurology and Memory Care
We are seeking a skilled and compassionate Nurse Practitioner who is licensed in New Hampshire* to join our virtual care team. This is a remote, patient-facing clinical role; delivering care to Local Infusion patients and partnering closely with physicians and the broader care team to increase patient access and reduce time to treatment.
You must live in and be licensed in New Hampshire and you will work remotely from NH.

Job Description
- Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth
- Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
- Partner with our neurologist(s) and broader care team to ensure continuity of care
- Educate patients and families on diagnoses, treatment plans, and disease progression
- Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements
Key Accountabilities
- Active, unrestricted NP license in New Hampshire; certification in geriatrics, adult-gerontology, or family practice preferred
- 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
- Strong clinical judgement and clinical documentation skills
- Comfortable managing a full patient panel independently with physician collaboration
- Excellent communication skills with patients facing cognitive decline and their caregivers
