Physical Therapist (PT), Full Time, Home Health

📋 Description

• Visit clients in their homes to provide PT care in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, ROM, gait, balance, strength, safety).
• Implement wound care and protective interventions with evidence-based dressings.
• Develop and adjust individualized care plans with manual therapy, gait/balance training, and other
• Document observations and evaluations using a state-of-the-art tablet and coordinate with nursing

🎯 Requirements

• Active and current PT license
• One year of clinical experience preferred
• Home health care experience welcome; training provided
• Ability to travel locally to cases as assigned
