Job Description
📋 Description
- Provide direct support to individuals with intensive behavioral and mental health needs using
- Support individuals per behavior plans, crisis-prevention strategies, de-escalation, and manual
- Maintain a clean, safe living environment; assist with daily living, transportation, and community
- Document services, observations, health concerns, and activities using electronic systems (T-logs
- Monitor behavioral changes, identify escalation indicators, and communicate with supervisors during
- Support family contacts, appointments, medications tasks (as allowed), and follow medical/nutrition
🎯 Requirements
- High school diploma or GED
- 3+ years experience with individuals with chronic/severe mental illness or a degree in
- Must be at least 18 (some programs require 21+)
- Valid driver’s license and auto insurance
- Basic computer skills and ability to use electronic documentation systems
