Job Description
📋 Description
- Follow designated care plan per patient's needs
- Make home visits to clients in designated territory
- Administer medications, wound care, treatments, procedures
- Monitor client conditions; report changes to manager
- Follow up on doctors' orders and document
- Perform client assessments as needed
🎯 Requirements
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Prior home care experience preferred, but not required
- Graduation from an accredited nursing program
- Ability to work independently and manage time effectively
- Strong interpersonal skills; solid computer skills with EMR experience preferred
