Social Worker

📋 Description

• Act as integral IDT member coordinating social work services for frail seniors
• Provide psychosocial assessments, care planning, counseling, and case management
• Collaborate with family, participants, and care teams to support safe community living
• Facilitate care transitions between settings and coordinate resources
• Lead discharge planning when participants disenroll

🎯 Requirements

• MSW required
• One year of experience with frail or elderly population preferred
• CPR and First Aid certification within 180 days preferred
• Experience coordinating care conferences a plus
• Strong ability to work within an IDT and with primary care teams
