Job Description
📋 Description
- Provide RN home health visits in Lebanon County, PA.
- Follow designated care plans with 1:1 patient focus.
- Perform meds administration, wound care, and procedures per orders.
- Monitor patient conditions and report changes to supervisor.
- Complete documentation on EMR and touch pad tablet.
- Coordinate care and case management with clinical team.
🎯 Requirements
- Current Pennsylvania RN license.
- Minimum 1 year of recent clinical (medical/surgical) experience.
- Home health experience preferred but not required.
- Graduate from accredited nursing program.
- Ability to work independently and travel to cases as assigned.
- Strong interpersonal and computer skills; EMR experience preferred.
