• Connects PACE participants to WelbeHealth care team
• Onboards participants to the PACE program smoothly
• Support participants through their WelbeHealth journey

🎯 Requirements

• High school diploma or equivalent
• Bachelor’s in communications preferred
• Excellent organizational and customer service skills
• Ability to present information to groups and leadership
• Self-directed, adaptable in dynamic environments
• CPR and First Aid within 180 days, certification preferred
