Registered Nurse, Home Health Visits

📋 Description

• Provide skilled nursing in clients' homes as part of a multi-disciplinary team
• Perform comprehensive head-to-toe assessments to detect changes in condition
• Deliver acute and post-acute care (CHF, COPD, wound care, IV therapy, post-surgical care)
• Triage and manage urgent clinical situations independently in the home
• Conduct medication reconciliation, teaching, and safety monitoring
• Document wound assessments and care accurately in point-of-care systems

🎯 Requirements

• Current nursing license in good standing (state unspecified)
• One year of hands-on medical experience with adult/geriatric populations in relevant settings
