📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, gait, balance, strength, and safety.
• Implement wound care and protective interventions.
• Develop and adjust individualized PT care plans.
• Collaborate with nursing on multidisciplinary care plans.
• Educate clients and families on rehabilitation and safety.

🎯 Requirements

• PA PT license (Current).
• At least 1 year PT experience.
• Program graduation from APTA/ACHE approved school.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• EMR experience preferred.
