Physical Therapist

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, strength, balance, and safety.
• Implement wound care and protective interventions with evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy, gait, and balance training.
• Assess and revise the PT care plan, collaborating with nursing and the multidisciplinary team.
• Educate clients and families to promote health, safety, and independent living.

🎯 Requirements

• VA Physical Therapist license.
• Minimum of one year of PT experience.
• Graduation from an APTA or AMA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
