📋 Description
- Provide one-on-one skilled nursing visits for adults and geriatrics in senior living communities in
- Assess client needs and develop individualized care plans.
- Administer medications and perform wound care and physician-ordered treatments.
- Monitor client conditions and report changes to management.
- Coordinate care with physicians, therapists, and the interdisciplinary team.
- Educate clients and caregivers to promote health and independence.
🎯 Requirements
- Current New Jersey RN license.
- Minimum one year of recent clinical nursing experience.
- Home health or senior living experience preferred, but not required.
- Strong communication and organizational skills; EMR experience preferred.
- Reliable transportation to travel across Somerset, Middlesex, Monmouth, and Union Counties.
