📋 Description

• Provide home health PT visits in Central/Upper Bucks County area.
• Make client home visits per assigned territories.
• Assess mobility, strength, balance, gait, and safety; document findings.
• Develop and revise individualized care plans; implement therapeutic interventions.
• Educate clients and families to promote health and independence.
• Maintain accurate records with EMR and state-of-the-art devices.

🎯 Requirements

• Current PA Physical Therapist license.
• Minimum 1 year PT experience.
• Graduation from an APTA-approved program or equivalent.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient computer skills; EMR experience preferred.
